ในช่วงแรกเกิด เด็กที่ป่วยด้วยโรคกล้ามเนื้อเสื่อมดูเชนจะมีการเจริญเติบโตตามปกติ แต่จะเริ่มแสดงอาการก่อนอายุ 6 ปี และมักจะมีอาการบริเวณกล้ามเนื้อขาเป็นส่วนแรก โดยอาจทำให้ผู้ป่วยมีน่องโต เดินได้ช้ากว่าเด็กในวัยเดียวกัน และล้มบ่อยเมื่อเริ่มเดินได้ พัฒนาการด้านการเคลื่อนไหวบกพร่อง มีปัญหาในการเดินขึ้นบันไดหรือลุกจากพื้น วิ่งได้ช้า และในระยะต่อมาผู้ป่วยอาจเดินเขย่งปลายเท้าหรือเดินท่าเป็ด
